My Card Doc does not collect patient medical records or any protected health information.  My Card Doc specifically solicits contact information from those individuals who "opt in" to receive information about future medical card processes once regulations are established.  My Card Doc may share voluntarily submitted contact information from registrants with medical card providers services based on applicable laws and regulations once established.
